Clergy Annual Safe Church Attestation
By filling out and submitting this form, you agree to the following statement. See Canon Section V, subsection VI, B for more details:
“Per the Safe Church, Safe Communities Policies and Procedures for the Protection of Children, Youth and Vulnerable Adults of the Diocese of Wisconsin, I attest that I am an active cleric in the diocese and that I am current with my Safe Church, Safe Communities training. I also attest that this year I have alerted and encouraged ministry leadership in my parish or organization to read this Policy and to complete appropriate training.”

Cost for each background check is $23 and will be completed by Prasidium. Invoicing will happen by the diocese after the end of each month to the requestor. Invoice will be able to be paid electronically. * Note, Commission on Ministry Background checks should be done with your COM team!Sample Request:
